To participate in HCA Virginia’s Diabetes Care Center counseling and education program, your primary care physician or doctor will need to provide us with a signed order of your diabetes diagnosis (a referral). Most insurance plans are accepted; however, you should check with your insurance provider regarding your coverage.
About the program
During your visits, a diabetes educator will meet with you to identify your needs. We'll discuss any questions you have, teach you the basics of diabetes and nutrition and help you develop a plan for managing your diabetes and staying healthy. Please eat before your visit, and arrive at least 30 minutes before your appointment start time so that you can register. Bring the following with you:
- A copy of your primary care physician or doctor’s signed order of your diabetes diagnosis
- A list of all medications you currently take
- Your blood glucose monitoring equipment, if you'd like help using it
- Your blood glucose diary
- Insurance information
- Photo identification
You are also welcome to bring a support person with you to your appointments.

The Diabetes Care Center is an outpatient diabetes program that requires a doctor’s referral to participate. Please ask your doctor to fax the referral to (804) 483-1755.

To schedule an appointment with the Diabetes Care Center, please call diagnostic scheduling at (804) 327-8706. We ask that you arrive 30 minutes before your first appointment to register with central registration on the first floor of the hospital.
Chippenham Hospital provides a full range of diabetes classes, customized training and services, including the latest in education for individuals with prediabetes, type I diabetes, type II diabetes and gestational diabetes. Our program to help you learn to self-manage your diabetes is recognized by the American Diabetes Association, and our team includes certified diabetes educators (CDEs). We offer medical and personal counseling sessions, small classes and support groups.

Our free diabetes support group meets bimonthly at HCA Virginia’s Retreat Doctors' Hospital. It does not require registration and is open to anyone affected by diabetes, including loved ones. Each meeting, the group discusses a wide variety of related topics, both medical and personal.
Our classes are small to accommodate individual needs and typically last two to three hours. Classes are usually scheduled over a period of two to three months and cover topics ranging from types and causes of diabetes to goal setting and problem solving.
The Diabetic Limb Salvage Program at HCA Virginia’s Chippenham Hospital is designed to provide high quality and compassionate healthcare dedicated to diabetic limb salvage through a streamlined, state-of-the-art multispecialty network.
